Quick Rundown:

The Intake Coordinator manages Home Health referrals from initial receipt through admission and transition of care. This role handles patient and insurance verification, authorization requests, utilization reviews, and referral coordination while ensuring timely admissions and clear communication among patients, healthcare providers, referral partners, and internal teams.

What You’ll Do:

  • Perform pre-admission, admission, and concurrent utilization reviews to determine the appropriate level of care and readiness for discharge or transition of care.
  • Receive and process new referrals, verify agency assignments and payment methods, and gather patient information, including services, treatments, medications, and physician orders.
  • Coordinate with referral sources, case managers, social workers, physicians, and internal teams to facilitate admissions, determine referral acceptance, and obtain necessary authorizations and co-payment information.
  • Verify Home Health eligibility and staff availability before communicating referral acceptance to patients, referral sources, and physicians.
  • Communicate with patients, families, and caregivers regarding Home Health services, insurance coverage, admission requirements, and related concerns.
  • Coordinate upcoming referral needs and anticipated discharge time frames with marketers, clinicians, and coordinators to support smooth transitions to Home Health.
  • Monitor interdisciplinary referrals and special assignments to ensure timely and appropriate use of services according to agency policies and standards of practice.
  • Serve as a liaison between referral partners and internal departments, respond to inquiries, resolve customer concerns, and provide feedback on received cases.
  • Maintain accurate patient and customer information in the ABS system, as well as detailed referral and lead-tracking reports.
  • Identify account growth opportunities and gather referrals and customer references from partners.
  • Answer office phones, provide on-call backup coverage when necessary, and report patient care, staff, or support-staff concerns to the Director of Nursing and Branch Manager.
  • Participate in in-service programs, staff meetings, and continuing-education activities, and perform other duties as assigned.

What We’re Looking For:

  • College diploma or at least two years of relevant experience in a similar role.
  • At least one year of relevant experience is preferred, particularly in healthcare intake, Home Health, or utilization review.
  • Strong communication, customer service, and telephone etiquette skills.
  • Good organizational, time-management, and problem-solving abilities, with attention to detail.
  • Proficiency in Microsoft Word and general computer applications; knowledge of Excel is an advantage.
  • Ability to manage multiple referrals, complex scheduling requirements, and time-sensitive tasks independently.
  • Familiarity with healthcare procedures, patient eligibility, insurance verification, and authorization processes is an advantage.
  • Sound judgment, professionalism, and the ability to prioritize urgent concerns and coordinate effectively with multiple stakeholders.
